First step would be to update to MainStage 2.1.2

Well I just tried in 2.1.2 and it still sends all notes off. Real bummer because I would play an intro with a pad sound and then hit start and it would stop the sound. Wish there was even a hack to changes this.

I have found that if you open the Playback plug-in and click the start button with the mouse it doesn't send the notes off message.
Not very practical but the only work around I have found so far.

You might want to contact Apple regarding this issue:

Well I have just the other day discovered the "Defer patch change". It works with playback plugin and does not send note off until you release the keyboard. Hope this helps.

From the online manual:

Deferring Patch Changes
By default, when you switch patches, the new patch is ready to play immediately. You can “defer” a patch change so that the patch change occurs after the last note of the previous patch has been released or sustained.

To defer a patch change
In the Attributes tab of the Patch Inspector, select the Defer Patch Change checkbox.

Note: Deferring patch change works in Perform mode and Full Screen mode but does not work when you are editing patches in Edit mode.
