gwales
I am almost certain Logic is corrupted. I have two keys intermittently failing to trigger MIDI, but it happens on different keyboards (same keys). When I use my MacBook Pro, no problem. Any advice? Do I just reinstall, or should I try removing preference files in the Library first? Hmm, no preference files? I'm going ahead with reinstall. My Roland seems to be responding OK now but the Yamaha SY85 only triggers 100% on the MacBook Pro. What else could be causing the problem on the Mac Mini? A corrupt MIDI driver inside OS X? I've tried starting a new project, in case a project was corrupted.

gwales
Choose Logic Pro X > Control Surfaces > Bypass All Control Surfaces, does the issue go away? It has. Thanks very much. I was quietly going insane. I'm using an iControl to enable MIDI to USB. It works OK with my MBP. I also see that every time I connect the iControl it makes a new instance (icon). How can the iControl work if I bypass control surfaces?
